Synchronicity refers to  “meaningful coincidences” that seem to be connected by something other than chance or logical cause-and-effect relationships. Where the connection between the events holds personal or symbolic significance. Along with the psychological significance these coincidences often align with a one’s inner thoughts, feelings, or current life situations.

Program Details:

  • Length of Stay: One year, with the possibility of an extension up to a maximum of 18 months.
  • Accommodation: Independent, furnished units with rent geared to income or the shelter portion if on income assistance.
  • Support Staff Onsite: Monday to Friday, 10:00 AM – 6:00 PM.

At Synchronicity Second Stage Transition House Program, we guide participants through our Circle of Care Case Management Tool—a framework designed to support survivors of intimate partner violence in building a foundation for healing, growth, and independence.

The Circle of Care focuses on five key components that address different aspects of life after fleeing violence. Through dialogue and reflection, participants are encouraged to answer meaningful questions about their goals, needs, and aspirations in each area, creating a personalized path forward.

1. Safety Planning

Safety remains the top priority in every stage of the journey. This component focuses on:

  • Identifying risks and developing strategies to minimize them.
  • Creating a plan for physical, emotional, and psychological safety.
  • Building confidence in navigating systems and accessing resources that enhance safety.

Ask Yourself: What does safety look like for me and my children? How can I feel more secure in my daily life?

2. View of Self

Reclaiming a positive self-image is a powerful part of the healing process. This sphere focuses on:

  • Building self-esteem and confidence.
  • Exploring personal strengths, passions, and goals.
  • Celebrating growth and progress along the journey.

Ask Yourself: How do I see myself now, and how do I want to see myself in the future? What can I do to nurture my sense of self-worth?

3. Health

Holistic well-being is essential for healing and growth. This sphere explores:

  • Physical health, access to healthcare, and regular medical check-ups.
  • Emotional and mental health support, including counseling and self-care strategies.
  • Developing routines that prioritize overall wellness.

4. Relationships

Healthy connections are vital for rebuilding trust and support networks. This component emphasizes:

  • Exploring and redefining personal boundaries.
  • Building supportive relationships with family, friends, and community members.

5. Finances

Financial independence is a key step toward long-term stability. This component focuses on:

  • Creating a sustainable budget and understanding financial resources.
  • Accessing education, employment opportunities, or vocational training.
  • Building financial confidence and planning for the future.

The Circle of Care Case Management Tool is not a rigid structure—it’s a fluid, evolving process guided by the participant. Every step is taken at their own pace, with support from program staff who are there to listen, guide, and empower.

Expression of Interest Form for the Synchronicity Second Stage Transition House Program

Who is eligible?

  • Self-identifying women (with or without children) impacted by relationship violence
  • A referral from a first-stage transition house
  • Indigenous, Black, racialized, immigrant, newcomer, and refugee families seeking support
  • Up to two companion animals may be accommodated under program policy

Commitments in the Synchronicity Program:

Participants are expected to:

  • Actively engage in the program
  • Attend weekly one-on-one meetings with the Synchronicity Coordinator
  • Participate in workshops, activities, and special events with their children
  • Join the weekly Women’s Support Group to build skills, communication, and empowerment
  • Receive ongoing emotional and practical support, education, and advocacy

  • If a suitable vacancy arises, we’ll contact you for an interview and application form.
  • Update your Expression of Interest form via email only (phone calls cannot be accommodated).
  • Forms are kept on file for one year.
